My girlfriend is a nurse and no they couldn't care less, they have seen everything.

Many guys go commando. If they do have underwear, it has to go down to the ankles anyway, or she sometimes has them disrobe and put an examination gown on which is open in the back.

She also sees many guys shaved smooth when she does the genital and anal exam part. She actually likes that better as she can see the testicles a lot better when she examines them.

Doctors can handle naked people, they have seen it all. So it's all about the patients comfort to keep underwear on. So I would say he is fine skipping, if he is fine being naked.
