Well, it sounds like it could be an allergy reaction. it could be that your body has built up an immunity to the zyrtec and it's not working any more. that's one theory. though, I highly suggest you see your doctor. Have you just looked up "swollen lymph nodes"? I did and this is what I got: infection, virus, inflammation, cancer, cancer of the blood. Dont' freak out at these things, there's no point in freaking out. Just go to your doctor and see what he says. If you end up needing surgery, then you should get it. You shouldn't let your fear of surgery stop you from trying to get better (I know, I was in the same position, and if I didn't get the surgery I needed, I would be eating out of a tube for the rest of my life). I know that what I wrote was somewhat callous, and I apologize. I hope things get better, and good luck.

the spread of infection could be due to metastasis of the localized infection that you can't detect, the small inoculum of infectious material may have spread and you are right to be concerned.
just eat healthy as best you can, boost your immune system and get to that doctor asap.
I really doubt it is an allergy based on your description. My first thought was mono, and you have already thought of that. It is also possible to have a tenacious strep infection for that long. If you have any kind of impaired immunity it could be lots of things. Your doc is really the best person to run the appropriate tests and make a diagnosis.
If the treatment calls for removing your tonsils it really shouldn't be that horrible. You should fully recover from the surgery within 2 weeks. Plus they are very good with anesthesia and have medications to help with your anxiety.

could be mumps(inflammation of the salivary glands) if it feels like you have a double chin that came from nowhere at all. but mumps is painful though...
just a note for you: if it is tonsillitis you should just get them removed. if you leave it for too long the bacteria (group A strep) that causes it can travel to your heart and lead to rheumatic heart disease. I'm not trying to freak you out, but better be safe than sorry.
