I used to have the same problem, it took a while till I got rid off it but now they are gone.
I would recommend accutane. Many people have told me that it makes the acne go away. I've never tried it though because I think I may be allergic to it. If you don't want to risk it then you can use toothpaste or hydrogen peroxide on the area. That's what I did and it worked for me.
The most important thing is to keep yourself clean.
Take a shower at least once a day and wash your body with a body scrubber and an antibacterial cleanser. DONT USE OILY SOAPS.
DONT USE HOT WATER WHEN YOU ARE TAKING A SHOWER.
When you use conditioner on your hair, make sure it doesn't get in contact with your back because that may cause the skin to break out.
Use a clean towel and make sure your body is 100% dry before you get out off the bathroom.
Wear cotton clothes it's better for the skin.
Last but not least stay away from chocolate, carbonated beverage and sugar. Maintain a healthy diet. Or at least decrease the amount of sugar you consume (that's what I did because it's extremely difficult for me to stay away from chocolate).
For acne scars, I know it sounds strange but I recommend using lemon or sunbathing. Lemon bleaches them and gradually it makes them disappear.

both my daughters had this problem in middle school...they use acne face wash (this one in particular worked/works for them both link ) on their shoulders/back as well as their face once a day and would spot treat with that little tube of Salicylic acid on their back/face. you can get it at any store pretty much just be consistant and always use that brand and EVERY DAY without fail, don't let yourself run out of it and it should clear it up. Also make sure you're changing the sheets on your bed and your pillow case every few days. If you exercise clean off immediately afterward even if it's just water you use.

Sounds like this is a serious problem for you and off the shelf treatments/washes/creams probably aren't going to cut it.

Using Witch Hazel can really help. My sister had the same problem and by her using Witch Hazel it really cleared up her back. Its not expensive and you can get it over the counter.
Wash your bedsheets and shower regularly.
