Assuming you are on a diet you can hit a weight plateau if you constantly eat the same stuff at the same intervals everyday.
Also sticking to the same exercises as a routine will make you hit a weight plateau.
When you hit a weight plateau stick to raw vegetables and/or fruits for a day or two this will help the body break the monotony and your body will again respond to the diet.
People go in for personal trainers and those guys never give you the same exercises everyday, this apart from ensuring that you don't skip 2 rep or two when you need to do a whole set. Variations in the exercising pattern aids immensely in both fat loss and muscle gain :)

Are you going on diets or something? Remember that in order to change your weight you have to change your eating lifestyle, you can't just go on a diet for a little bit then go back which would cause this.
You have to be persistent, and consistent with your exercise and caloric intake. I hit a plateau and it took me months to break it. Good luck you can do.
