When I got my wife pregnant with our first kid it was go mode to get as much done as possible before she popped. I just knew that with sleepless nights, and everything wouldn't be able to prepare for, the more we had down, the simpler and easier things would be.
That worked out for the most part, and seemed to make things a lot less stressful on her, both during her pregnancy, and being a new mom. I would hold off on the child gates though, she was a little pissed to have those set up on the steps while she was still having morning sickness lol

Whenever. Most parents I know have their kid in the same room as them until the baby is at least a year old.
We set up and decorated our nursery room some three/four months into my first pregnancy.
Now that our first born has his own bedroom, we redecorated it completely for our fifteen month old little daughter.
With my first I was 24 weeks and Second the room was already set up, I just need a cot although a repaint as my first scan told me I was having a boy

Anytime after the anatomy scan so you know for sure that the baby is healthy
I would say at least 6 months into the pregnancy gradually start setting up a nursery.
I'd start right away with a baby you just never know. Why wait im not into any of that superstitious stuff
About into 5 months due to miscarriages so then you have a empty room
My wife and I had 3 till one birthEarlier the better, I would think.
Honestly, there isn't much to set up, so I'd say there's no rush.
