My friend is in a long distance relationship?

So my friend is 23 and she lived abroad when she was 20 and met a guy whom she was with for 4 months. They lived together in those 4 months aswell and fell deep in love. They arranged that he would move to our country and they would live together here as she does wants to live close to family. They started the visa process straight after she left and due to Covid and visa complications they have been long distance for 3 years. They are still working the visa out and applying finally next month. But in the last 3 years she only saw her boyfriend 3 times. She visited him after 5 months the first time and then did not see him for a year and a half and then didn't visit him for another 6 months. She is very committed but I worry she might get hurt. How can they call this a relationship? Their needs are not being met.
