The first thing you need to realize is the difference between those who have worked hard for their wealth and those to whom it was given.
A general surgeon makes about $350k/yr, which is about what I make, not counting capital gains or dividends. Specialist surgeons can make over $500k.
To become a surgeon means starting one's career well into one's 30's. Those are many hard working years without a lot of pay and enormous loans to pay off.
I don't know what you call a mansion, my main house is 4,200ft2, but I also have two more houses (one on a tropical island) and a cabin in the woods, plus my airplane cost more than all but my main house. And in her case, it doesn't sound like it is her house, but her dad's? Her father deserves everything he has, and that includes the ability to send his kids through school.
Who is she ungrateful to? One doesn't have to be rich to spoil the little girl among of a brood of brothers.
Maybe you think she doesn't care about medical school, but in reality, maybe it isn't what she wants to do, but she is being forced into it. Have you thought about that? I went out with a girl whose parents were doctors and she was about to go to medical school and resented her parent's demands to do so. She was capable of doing the work, but hated every minute of it.
If these girls tell their parents they want to do something much different, say fashion design, they will be cut off. Now what?
Not everyone who wants to become a doctor can, and not everyone who does become a doctor really wants to. There are many motivations. Many are motivated by nothing more than the high salaries. Others are motivated by wanting to help people. Some are motivated because their parents told them they have to. That does not mean she is ungrateful, but it can mean she is resentful.
You said it yourself, she's spoiled. Rich peole are catered to. She obviously doesn't appreciate what she had. Rich people think they are better than everyone else just because they have money. But, that's just one side of it. You really want to know? Ask her. She may not even be aware of how she is behaving.
It seems as if there's some sort of "tradition" in her family. Perhaps her father/other relatives are pressuring her into medical school, and it really isn't something she enjoys.
Maybe she doesn't want to be a doctor and is just following in the family footsteps.

I think the problem is with you.
Take the focus on money away and what is left in this story? A girl with a busy single father and brothers all in demanding careers. She's about to go on the same path and isn't sure if this career is for her.
It's exactly the same problems that everybody faces. You need to stop being such a judgmental jerk.
Just a thought, maybe she doesn't want to be a doctor and is being pressured, what if she really misses her mom, what if she feels like the dumb one, what if money isn't important to her, maybe she feels unloved etc.
Entitlement... Why work if it will all be handed to me on a silver platter with a nice pretty bow on it
Adversity breeds character, and not worrying about money takes away A LOT of the adversity.
Some rich people are handed everything they want or need their for their entire life so they don't have any appreciation or awareness of the hard work it takes to be successful

She wants taught responsibility, character, integrity, love and the value of a dollar. For example look at Dave Ramsey's kids. He's a millionaire yet he kids turned out good. He taught them correct.
Well, there's a difference between ungrateful and careless. I can say people forget just how much they have of something after having it for a long time. (And not just wealth)
