I called her by her first name. I had zero respect for her after she legit kept rostering me on to work on Tuesdays when I told her before she hired me that I couldn’t work this day. worst of all, she than threatened to fire me after I fell pregnant and told her I couldn’t work on the Tuesday because of a medical appointment. Too bad for her she had no grounds to actually fire me because she would have been done for unfair dismissal.
I had a job when i was 19 working at T. J Maxx and my boss always wanted me to refer to her as Ma'am but she was a very nuce boss so i didn't mind or have a problem with calling her Ma'am

It depends on the boss. Usually, I will use their title and last name. If he/she demands that I use Sir or Madam, I will feel a bit annoyed, but will do so regardless. Customers/clients/stangers, however, are a different story - then it's either Sir or ma'am.

No. All first names for just about everyone up the ladder but in academics people get REALLY PISSY if you don't use their honorifics like "Dr".

UK here. I mostly call my boss by their first name. If they're super authoritive I would call them Mr. Surname but that is very old-fashioned.
