So be honest i've always wanted to know is it honestly an accident?

Depends how big they are. If you have big boobs, a normal t-shirt would already give cleavage. Only a turtle neck or something else with a really high neck won't show it, but those shirts often look awkward with big boobs.
So you can either put on something that looks a bit awkward or something that shows cleavage.
Other than that, pictures, shirts with really low neck. Those are often on purpose. But does it really matter? It's just a bit of cleavage. No harm really.

I don't like showing my cleavage so if it ever does show it's an accident for sure. I know girls with bigger breasts can't always help it though. My sister has D cups and hers accidentally show no matter what she wears. But some do do it on purpose so every girl is different. If it is a lot of the breasts showing then it's usually on purpose. If it's a little cleavage then it might be accidental. Hope that helps.
