It has happened to me once and to my 2 female friends. We were 16 year-olds at the time but he was mature for his age. I actually did fell in love with him for both his looks and other traits such as a good, family-oriented boy, he was kind to others and liked reading and liked animals. He thanked me, said he felt flattered but wasn't interest in a romantic relationship, though we can be friends. I could tell he had a hard time saying all that.
If he would've rejected me harshly, called me bad names, acted grossed out or made a scene out of it, yeah I would've hated him forever or probably had self-esteem issues. It still hurt but much less than if he had been rude. We're still friends till this day and both of us dated others.
This happens to guys too. Who do you think gets hurts the most? The man or the woman?
