How do you feel about people who insist on being called "Doctor" instead of Mr/Mrs etc?

I'm not talking about people who are actually medicinal doctors; I'm talking about people who have achieved a doctorate in their subject.

I understand that earning a PhD is a very hard task, but saying "It's actually Dr John Smith, not Mr John Smith" all the time, and getting annoyed when people dont call you Dr.. it just seems a bit... arrogant?

Or should they have the right to get annoyed and correct people?
