Ugly is subjective but I’m going to assume if you rarely get compliments on your appearance that may be a sign. Also people like to hang out of speak to people they find attractive more. So if you get ignored often or people seem uninterested in knowing you that may be a sign. Also if people rarely look at you. People like looking at things they think are pretty.
No. The people who are mean and harsh are the ugly ones, even if their outward appearance is fine. I have met very few aesthetically "ugly" people in my life and almost all of those were such nice people that it would not matter much to people with any depth of character.
No people can be terrible to other people. So don't take people's opinions to heart
