I’m needing some advice on how to approach an attractive guy from my gym. I think he’s interested, I’ve caught him looking my way on multiple occasions, sometimes multiple times on the same day. But I’m out of practice and don’t know how to approach him and what I could possibly say. Help me out here, how do I approach him and what do I say?

A bit strange but here's a possible tip: walk past the guy and drop something on purpose and keep going. They will make a move if they are keeping an eye out.
If they don't, they are either not interested or not confident enough to talk to a girl.
And of course there is always the direct approach. Not a single guy has not dreamt of a girl approaching them. So even if things don't work out, he will remember you for as long as he breathes. :) Have a good day.
You can just say hi and introduce yourself, He is more than likely more afraid of Approaching you than you are of Approaching him.
