How do I approach a girl at the gym when we barely know each other?

Don't unless you're 120% sure that she's interested in you (gave some very obvious hints). People go to the gym to workout and not to date and having guys hit in you while you try to workout is annoying and one of the reasons for women only gyms.

have checked her out and pretty sure she noticed but she's came like rate near me to work out on 2 separate days , once when i was on this machine and today when at hand weights and she was also fairly close nearby again when i was on that machine , she was with a girlfriend not by herself

You don't. This is why many gyms have 'female only' hours.

my gym doesn't do that although do wonder if it might be something worth considering , although think a lot of women here go to a smaller gym ( that i've never been to ) or yoga/pilates places
