I think it depends on the situation. If it's just some random IG girl, then no. If it's someone he or both of you know, then maybe, especially if it's the same person all the time. If it bothers you, I would ask him about it.
Only worry if you have nothing that sets you apart from all of those other girls.

The important part here is the "in a HAPPY relationship". Can't tell you if you are but the cheating doesn't scream "happy" to me.
Instagram likes don't matter, a history of cheating is a red flag though.
