I'm killing myself to get this weight off. I need to lose over 180lbs.
What more do I need to do? I hardly eat any carbs like less than 20g to be in ketosis and I'm getting lots of protein in.
I'm no expert but I think you're doing ok, all you need is more time, and the results will show.

• Unless you didn't eat for a month & have a BMR to Sedentary TDEE (Total Daily Energy Expenditure) range of calorie burn that's above 2,500cals, then some of that weight-loss was just from water weight.
• Sadly, moving more doesn't always translate to long-term reduction in body-fat due to exercise induced energy-compensation. Unless someone's daily averages in EAT (Exercise Activity Thermogenisis) surpass what their usual daily EAT & NEAT (Non-Exercise Activity Thermogenisis) burn averages are, then their metabolism will very likely stay the same long-term.
• Diet type doesn't matter for fat-loss. Diet type simply changes what the body oxidizes in the moment. It doesn't increase or decrease metabolism significantly on it's own. What matters for long-term fat-loss is that you're calorie intake averages are lower than your calorie burn averages.
• Decreases in body-mass come with decreases in metabolism. So as you lose weight, you'll have to keep calorie averages proportionate to whatever your former or preferred rate of weight-loss was/is if you wish to continue to lose fat at the same pace.
Set little goals at a time. If you get obsessed with the end result, you'll get frustrated at some point. It's also easier to get weight off at the beginning, and it gets harder the further you go.
Instead of setting your goal at -180 lbs, set your goal at -20 lbs from your current weight. That will take several months, whatever seems feasable and reasonable. Starving yourself is not going to get the long term results you want.
Once you've lost those extra 20 lbs, set your new goal at -160 lbs and work on the next 20 lbs. There's no miracle solution, you are reverting eating habits and lack of exercise over a longer period, probably years.
