Need to lose weight and gain muscle--advice?

I need to be able to pass a physical fitness test by September. I'm currently at 195 pounds. I don't necessarily have a target weight but I figure around 130-140 should be fine for my height (5'4"). I really just need to gain more muscle and the weight should theoretically shed as I go along.

Anyone have any tips on how to begin? What gym equipment I should be using? I'm planning on working out for an hour six times a week.
