If somebody is willing to pay thousands of dollars to change the way they look its obvious they must be very unhappy or insecure.
No.
Many people require plastic surgery for different reasons, even medical. Like deviated septum makes it difficult to breath and can impair air flow, so a nose job may be essential. Or a cleff lip may need repair, so plastic surgery helps hide the scars and surgical repair that was done at a young age. Burn victims may choose to have their burns covered by plastic surgery / skin graphs for helping them recover and heal faster, etc.
The thing about COSMETIC plastic surgery is society has deemed this ONE prominent ideology of beauty, which is now small upturned nose, cheek bones, doe eyes, etc. Plastic surgery done for cosmetic reasons doesn't mean someone is mentally ill, it just means they desire a change to feel more confident within themselves on the outside.
Some people get bullied for their whole lives about how big their nose is, their big forehead, their chubby cheeks, or non-existing jaw. Kids grow up being bullied, and it completely effect how that person sees themselves, which is why so many choose to undergo surgery, as society and other people make those things insecurities that truly harm someones mind and self-worth.
There ARE mental health disorders that can absolutely push people into choosing plastic surgery too, like BPD, or Multiple Personality Disorder, as these people experience seeing themselves in a body they may not like, or may not feel truly suites their mental outlook on themselves, so they choose to make changes to their body surgically, to feel more confident in their own skin and BE the person they are imagining / desiring to see.
Plastic surgery is done for so many reasons. It isn't indicative of mental disorder / mental illness just because one chooses to get a nose job, or some lip plumper as a way to make themselves feel better about themselves. :)
