Presumably, she says no. Or "not now" or "I'm not ready yet", or some variation thereof. Then the couple decides what to do next.
The "general rule" you mention describes what is a common scenario; perhaps the most common, but it's not everyone.

Inconceivable? You're talking to the guy (make that idiot) who proposed twice to the same woman and was turned down both times. Yes, woman can say no, and they will.
This can be avoided through conversation and knowing where each other stand. The marriage discussion should happen before the proposal; when the man finally proposes the woman is excited and not in fear.

Relationship is a sham if she's not ready. If the man is a good man w good intentions and the woman is a good woman w good intentions the woman will never be not ready before the man. Period. Anything else means some fucked up ness bullshit is going on.
If she's not ready when I ask, she's not worthy of my time, and she is wasting my time. I would ditch her instantly. Thankfully I'm married and I don't have to worry about it.

If a woman was raised by a successful single Mom, they don't learn the need of a marriage. Could be many other reasons to be not ready.

Because you just said so yourself. Women tend to always want to be getting married, so, it's seen as "different" when they're not. Why wouldn't they?
