I think that's kind of the myth part.
I think the white under the eyes relates to excessive 'yin' in Chinese medicine. It's all to do with the nervous system and how that indicates health.
You should be able to correct this by reducing yin and becoming more yang.
I notice I have more white under the eye if I'm more yin.
Conversely, they say people who have white above the eyes should not be trusted, and may be quite evil. Pretty superstitious really. But likely has some relation to truth somewhere!
If you're interested in reading faces then there's some books by Kushi called your face never lies. There's one for body, too. This topic is covered in depth. I definitely think there's merit to it. People would say it's superstitious, and likely is in a bastardised form. However, the Western approach is clearly not getting the nub of health, looking at us critically and objectively.
You can say yin people probably occupy a more fantasy or spiritual world in general. Whereas Yang people are more worldly in orientation. It makes sense that the latter would generally be more physically robust because their occupations are worldly. They tend to strive and condition themselves physically whereas yin people tend to be more passive. However, clearly, there are many yang diseases. In fact, they're generally the Western diseases of affluence. In short, their health problems are generally different.

So much of Sanpaku appearance has to do with head angle and where the photographic subject is looking. I think Sanpaku is not a reliable indicator of destiny, or mental health or condition. May you live at least as long as my Sanpaku eyed grandmother, who still lead a healthy if uneventful or tragic life up to age 95.

As for the Sanpaku bullshit, it's just how you hold your head. If you look in the mirror you can adjust your head and expressions to give yourself "sanpaku".
