Can I get a bigger picture of this, so I can break down the numbers and see if that's what they really did? It's before the subtotal and then tax after is a certain %... I think they actually taxed a tax.

What the hell is "2 water donation"? Fuck California and their bullshit.
I think the health insurance is another word for the tip but itās less likely to be contested and refuted if itās viewed as a health payment and not a luxury payment for the employees beer or car or cat food etcā¦ itās just a marketing term that replaces the word, mandatory tip.
$19 for lettuce on bread? The restaurant can afford their own health care with those prices.

I would give zero tip if I ever got a check like that. It's the restaurant's job to provide the staff with health insurance, not mine. If they want to raise prices to do that, fine, I can decide whether I still want to eat there. But don't just pass it off to me like it's my responsibility just because I patronized your business.

I would ask the server if they had health insurance. If they do, I would as if they have to pay in too. Depending on the answers, I would take the appropriate act on Yelp or some other forum.
It is weird, especially for a place like Alimento which is already silly pricy. $1.50 for a glass of (maybe purified) tap water? Come on, now.
