If a guy opens and/or holds a door open for me, I smile, make eye contact, and say 'thank you.'
If I get to the door first and someone is fairly close behind me, I'll usually head in first, but hold it open until the guy [or anyone, for that matter] gets to the door, so it doesn't slam on them. I would never stand there and wait for the guy to come up and expect him to open the door for me. I would feel like that's rude, even if the guy -wants- to open the door for me. It would be rude of me to expect it, if that makes sense.
I don't EXPECT a guy, or anyone else, to open or hold a door for me, but I always show appreciation if they do. And likewise, I'll always hold the door for someone else if they're behind me. It's just good manners.
I don't really follow those kind of 'traditional' manners, like the guy should always open the door for a girl, should pull out her chair for her, should ALWAYS pay for her meal, etc. One thing my dad does that I think is sweet is that if I go somewhere with him in his car and we're getting in, he'll always come around and unlock, open, and close my door for me before going to his side. All of those things are nice gestures, but I don't expect them from anyone, and I think I would be kind of annoyed if guys did those things for me ALL the time. Like I know in my head that it's an etiquette thing that will probably never completely die out, but my instincts tell me that I need to be more independent, because I like equality.

lol, if you think he's cute you hold the next door shut so he can't pass through it and say "I got the door, but it'll cost ya" haha, that'll give you time to flirt. . .
If you don't like him, just say thanks and keep going, who cares if he thinks you're high maintenance. . . you'll only know him for 4.3 seconds, lol.
open it yourself... the guy... being a gentleman since he opened the first door will reach forward and grab it holding it as you walk through and then walk through himself... don't stand and wait.. that's prudish...
