Guys please let me know what would be my best approach, because I dont want to come off as needy or look like I am not confident if I check with him to see if the date is still on. I noticed he texts me less too. I hope he isn't dating anyone else.
Needy? If any guy is gonna consider this an issue or get turned off because that fact he needs to date a guy then. You should always tell him what day works best for you weather because of this or anything else and pretty sure he will be okay, that’s why you ASK for a date.
I would simply arrange the day right away, and no confirm.
Whether you're a leader or more submissive... if it's meant to be it will be.
If he gets turned off because you're too this or that it's not meant to be. Just BE yourself.

If he isn't putting much effort into it and doesn't care much, I wouldn't bother being nervous about this date happening or not.
If he wants to, he will make it happen, if not then oh well someone else will
