I never do! I just cut or tear off the moldy bits and use the rest.
My grandmother (years before I was born) used to clean out the fridge once a month and shed cut off all the moldy bits and toss the rest into a pot to make stew out of and the whole family ate that for dinner. Except for her hubby, who had a heart attack the year before I was born, every one of them (all 7) lived well into their 80's and 90's!!

Yes. Because mold has spores. Even if you don't see the mold yet doesn't mean it's not covered in mold spores.
Usually yes. If you see fruiting body of fungus it means its mycelium (roots) infested entire bread already. Not all molds are poisonous, white mold that turns in green/blue sometimes is harmless and grows on Bree cheese or specific sort of sausages. However I wouldn't risk it with bread.

Nope. Toss the slice out and smell the rest. If it smells yeasty, toss it. Otherwise freeze it until you're ready to use it

I'm a big fan of keeping things well past their sell-by date, and mould on cheese isn't a problem, just scrape it off, but moldy bread is a no-go. Chuck it, it will be all though the loaf, not just the bits you can see.
Unless it’s something you can cook or fry, throw out the complete food item. Don’t risk it, mold can make you violently ill.
If you can tell that the other end of the loaf is clean on the inside and exterior, it's safe to keep. But if you're not sure, just toss it out.
Mold spreads quite rapidly. It's usually not safe to do so.
If you understand how mold and fungi grow, then you would know you need to throw out that bread.
