He keeps forgetting my name?

So basically I've known this guy for a long time. Known as in we attend the same Church. And I've talked to him a few times. But whenever I get a chance to talk to him he introduces himself and asks my name. Within 2 years he asked me around 4 times LOL. Am I just easily forgettable? Is it possible he actually forgets. Or does he care so little to remember. He hears my name, his family and I are good friends and I dont get it haha. Why?
(I really like this guy too)

Oh man.. that guy either has serious ADHD or just a bad memory. He might like you a lot and try to hide it, but forgetting your name 4 times..! That's a lil' too much, don't you think? I wouldn't say that you're "easily forgettable", there's no such thing. You should ask him "ey, it's been about 4 times that I talk to you and you just can't remember me. D'you have Alzeimer or something?". Don't take it personally, but ask him with a joke or nicely. You should get your answer to your question (no, it's not scary.. go for it!).
