Is it true if a guy teases you playfully and tries to annoy you means he likes you? he's 6ft4 and im 5ft he's been calling me "little midget" Like he would message saying "i like annoying little midgets haha 😉😂😂" And when we hang out, he pats my head and taps my shoulder and arms Etc

Not always. I may LIKE the person as a person, but that doesn't always mean that I'm attracted to them in "that way."

I pick on some dudes (albeit a little differently) like they're my brothers. I'm just playful and sarcastic. If I didn't mess with someone in little ways, I either wouldn't know them well enough to know if they could handle it, or I wouldn't care much for them.

It MAY mean that he thinks of you only as a sister (I have friends like that), or it could mean he's interested. Or he just likes messing with people to get a rise out of them. Or to shake up the status quo. It's not an obvious thing. But it's a possibility.

In your case I think it's true, especially since he's being a bit physical by patting your head or tapping your shoulder. Guys tend to be pretty careful with that stuff and usually aren't very touchy with people unless they really like them. Whereas many girls can be touchy with pretty much anyone and even hug people they barely know.
In other cases I think it just depends on the guy, some guys are jokesters by nature and will poke fun at almost anyone.

Yes many guys do this, they stop the pulling hair in Junior High School and move on to teases you playfully about something you do or have done, your height, appearance but they always make sure you know it's all in fun, they have no I'll intentions.

Its all about context and chemistry. If he's playing and paying attention to you, then it means he likes you.

But thats not enough information to see if its sexual in nature.

You could always test the water by seeing how much touching or contact he's comfortable with.

Um this sounds a little mean. Like I would playfulle tease a girl about something stupid she did, but always in a nice way that won't break her down.
Calling someone a midget doesn't sound playful.
Maybe: is the guy under 15 years old, or a childish grownup?
Maybe he just doesn't understan the difference between playful and abusive.
In any case, sounds like he's into you, but be careful for the way he treats you.

It is a well-known fact in guy culture: Guys complement each other by insulting each other.

My experience is, a lot if guys (like myself) don't often extreme that to women. Many, like the guy mentioned in the question, actually do, and that's alright. Based on what you're saying, he definitely likes you.

It depends on the guy but I know plenty of guys like this so that may be a pretty safe bet. I don't know how girls put up with those sorts of guys, I feel like if a girl did that to me it'd just drive me nuts buuut... hey if you enjoy it then maybe something will come of it? I honestly have no idea but again, as a general rule I'd say this is a sign of affection.

He might be, he might not be. Society's pretty garbage at providing examples of good flirting behavior, so he might just be being friendly, or it might be his rather heavy-handed way of showing that he's interested.
