im a bit of an introvert and can be quite dense at times, sometimes someone looking at me might indicate somethings up, egg on my face kinda moment i'd suppose.
Where and when did you lock eyes with him, if its in classes or something along those lines it might be that he doesn't want to look stupid in public or something?

A girl I work with makes really strong eye contact during conversation, which is not normally considered flirtacious, but can be weird.

Shy? or some men are just inexperienced/dense and don't pick up on choosing signals. Saying that, eye contact doesn't really mean anything unless accompanied by other body language. So just staring doesn't give the green light to approach.
If you want to be obvious, look a man up or down, or when he looks at you look down and smile. It shows submissiveness.
Because it's too good to be true. If u are never called sexy all ur life and then just some random person is looking at you, the first thing on ur mind won't be that she's looking at u cuz she thinks your attractive.
Who knows. Maybe he's seeing someone else, maybe he's not attracted to the girl, maybe he's shy, maybe his mother is in the hospital and he's not in the mood
Because I'm not interested anyway? Being attractive is not some binding contract for every guy to approach.
