Maintain a good night routine. Stay away from heavy foods 2 hours before bed. Make sure when you exercise you do so at least 4 hours before bed, this gives your body time to slow down and not still have your blood pumping. A hot shower before bed can help as well. Make your bed for sleeping only. Take EVERYTHING out and away from your room. No tv, no cell phone, no ipod, no reading books, no eating, no lounging in/on your bed. ONLY SLEEPING. By doing this you'll create a better connection between your brain and sleepy time.

You might actually have a medical condition. I would suggest telling your doctor and having him refer you for a sleep study.
A new mattress will do wonders if you get the right one for your preference. (I. e. hard or soft)
Turn off your cell phone and computer and do not touch them for a week
exercise 2 hours before you plan to go to bed.
