What can I do differently this time to reach my goal weight?

Ok we all know, eat healthier/less and work out. I think I'm quite lazy and go through periods of motivation but after a while I'm like ugh i don't want to work out! I've been to fat camp and my weight has gone up and down drastically over the years. It's sad because I look back on pics and am like omg I wasn't even fat. However now I'm at my highest weight and I need to do something different if i want it to finally work. Could I be addicted to food? Should I go to therapy or overeaters anonymous? I just want to be more fit and not fat but I keep doing it wrong. Any suggestions?
