I've been stuck at 200lbs forever now. Can someone help me cut weight?

I think my ideal weight is like 20 pounds lighter than now, but i need to lose like 30 pounds of fat and fluid and build back like 10 pounds of muscle. I am eating a lot healthier than I used to eat, but still having trouble cutting weight. My stupid Type 2 diabetes and high blood pressure makes it hard to train the way I'd like to train, but I told someone two years ago that i wanted to try to get in good enough shape to fight professionally even though I'm really too old to be starting out. If I could cut weight to 180lbs again, I could kick ass in the middle weight division.

I really need hours after hours of advance cardio kickboxing workouts, I know that, but its' just hard to keep pushing myself because of my medical problems.
