I've worked as a waiter for years in various restaurants. I think I can give you a more insightful answer than most (since waiters and restaurant staff ALWAYS talk-shit about the people at their tables).
The answer to your question is: It depends entirely on the woman, and how she presents herself.
If someone seems shy, meek, apologetic or otherwise socially awkward based on a (at least seeming) lack of confidence in herself... then... yea... it would be more the 'loner' option. That being said, if that girl seemed nice, it wouldn't be assumed she 'has no friends'. Just that maybe she's an introvert, or enjoys her own company.
Most of the time, someone isn't going to come-off as an introvert. If you seem more-or-less-normal and/or seem confident... nobody will think twice about the fact that you're there alone. The trick is; to not SEEM self-conscious or uncomfortable with the fact that you're eating alone. If you treat it like "nothing"... then it isn't something people will think much about. 🙂

Strong independent modern women = To miserable lonely single feminist that has no children has a job that needs a lot of hours and probably has a pet, very likely to be a cat as those when they meow have the same frequency as a baby.
My suggestion to not be part of that lonely group. Get a husband have a baby and be a stay at home mother. Your life, emotions and wellbeing will be far better if you devote your life to family then to a boss who doesn't care if you die.
Our entire being is centered around family. Men provide the stuff women need to feed the children. If we had a small village in where the only thing done was harvesting food, chopping wood and gather resources then women would be looking after the children and make sure the village was clean and the buildings where tidy while looking after the kids.
There is nothing difficult about it as that's our basic human programed life. Some greedy corporation wants to make you a wage slave so they can fill in their pockets.

Some people like eating alone, myself included, so I don't judge people when they're out alone.
I chose B, but it could be a multitude of things, and my gut reaction would depend upon how she is dressed and how she is acting. Maybe she works in a small office and has a 30 minute lunch. Maybe she was meeting someone who had to cancel at the last minute. Maybe she is painfully shy. Maybe she prefers to eat in peace.
If she was eating a 20 oz steak, I'd assume that she's not a vegan. But normal people don't make wider assumptions based on that little information.
