Lets say someone gains 40 pounds or so. Is that a good reason to break up with someone?
Not just break up out of the blue. I think there should be what we call down south "a come to jesus" talk, but really... I mean 40 lbs didn't happen overnight where was that conversation before? If she's just actively decided to say F it and let herself go... well... I can't say i'm not going to let her go as well.

Depends on each one's lifestyle and how that effects the others ability to interact with each others lifestyle. For some it's no big, others it may be a concern if the other person is quite active and that is a big part of their lifestyle

But it's a good reason to ask about the health status and find a doctor who would do a detailed check up.
If it's not health related, you can together think of diet changes and a sport you can do together.
It could make your relationship only stronger.

Yes. It's indicative of other things going on. Of course, it could be due to some medical issue, but a lot of times weight gain is due to laziness. Like all relationships, you should start with talking about it openly but if she doesn't have the self-control or self-respect to change her habits to be healthy, then why would you want to be in a relationship with her at all?
You don't need a "good reason" to break up with someone. If you aren't feeling it, end it.
