Probably thinking they are flirting but they are not. That is implying that you make a move and flirt back and she shoots you down and you feel foolish.
The 2nd scenario you list implies you never know she was flirting. You'd be ignorant of it ever happening and you'd regret nothing because you never knew and never realized you had the chance to flirt back and see if it goes anywhere.
Now, if you mean you someone find out she was flirting and missed a chance then that regret would probably be worse.

An interesting question. Both are embarrassing, of course. No one likes to feel like they’ve missed the mark on a conversation. The one I would choose to regret would depend on the person I’m chatting with.

I don't think either are a big deal at all... it is just flirting
I admit to having been oblivious at times when someone was flirting with me. My now wife finds it hilarious that I don't or didn't catch on earlier in our relationship.
