Smoking could be a big reason why you're experiencing this. You need to brush your teeth 3 to 4 times a day after each meal, not only twice, because in your case twice is not enough. When brushing your teeth, brush your tongue as well, removing bacteria from your tongue can help reduce bad breath. Keep your mouth moisturized by drinking plenty of water. As far as diet goes, you want to avoid foods with a strong odor like onion and garlic. Red meat causes bad breath too, so if you consume a lot from it, reduce your red meat intake. Eat lots of carbs and vitamin C. It's good you're using special toothpastes and mints. I suggest using mouthwash, breath spray, and flossing, I think they're more effective than chewing gum. Never give up on your oral health, and everything will be fine sooner than you realize. :)
