I'm not a health expert however will make some suggestions which may help.
Firstly you may be sleeping with too many pillows so you might want to reduce the number to around one/two, so your neck is not so high up when you are sleeping, also try to be slightly conscious about the position that you are sleeping in. The reason I say this is because you state the pain is normally when you wake up.
Secondly, think about the activities you do during the day? Could this be causing it, if so again you may wish to change your posture during the day and take frequent breaks so that you are able to stretch and not store up too much tension in your shoulders. Try also deep breathing exercises.
I don't know if you are in the UK/USA?Elsewhere but what you can also do is purchase some heat wraps to put around the area they work wonders. You can also use hot/cold compresses in a similar way. Buy some gel to apply to the area you can buy the stuff that is not so smelly.
If you are working out a lot you might want to give it a break for a couple of weeks and see how you feel, or cut down slightly.
I would also suggest that you try some relaxation exercises, you can pick on various techniques found on places like you tube.
Do a search on the internet to show you exercises that you can do, to help relieve the pain.
Lastly, if you can try to save some money and go and see a professional, but you might want to try things at home and see how you get on, however you need to be consistent with whatever it is you have decided to do. If you can also get yourself booked in for a massage at least once a month.
A natural remedy, purchase some olive oil (you can get it from most pharmacies -it is for use on the skin), pour a small amount in to a bowl and microwave for around 20-30 seconds, you will know yourself when the temperature is right, and massage slowly where it hurts, DO NOT APPLY PRESSURE on the area. However first thing in the morning you need to start stretching, even if you start of by stretching in the bed first. I know it sounds weird however I hope all this information has been useful.

it sounds like it could be neuromuscular disease or tendonitis . also I never had any pain like that and I've been vegatarian for 7 years now so I doubt it has anything to do with that but then again being vegatarian could affect people in different ways. I would find out if there's a way you could go get it checked out without insurance some places you can go and they will work with you by your income where you won't have to pay that much . I know you have to be in a lot of pain and it could end up getting worse . I have tendonitis in my wrist and it ain't no fun at all
