I was with this girl awhile back who was a little heavy. I liked her a lot but she wasn't willing to try to lose any weight or try to get in shape, and when I started inviting her to bike ride, go to the gym etc with me she called me an asshole. Maybe I went about it the wrong way, I don't know but that's what happened.
It's not that I was bothered by her weight per se, but I do lots of outdoorsy athletic things and if she just wants to sit at home surfing the internet all day instead of at least putting in a little effort it says a lot about how a serious relationship might go.
Yes, but much less so than girls do. As long as a girl is reasonably sized (like... not 5' and 200 pounds), each guy will have his preference but not be too picky. Girls freak out and criticize themselves and others for equating 3 extra pounds to "fat".

I really don't care about the girl's weight... as long as she's happy with it (and it don't pose any significant health risks), and she's happy with me... it's all good
I think that they must look healthy granted I like skinny girls but they still need to look healthy and not anorexic.
