My grandparents had to move to an assisted living place because my grandma's dementia is getting quite bad. We are trying to get another doctor for them because the one that they have at the place is in-and-out in like 2 minutes, and doesn't seem to be very thorough or have a lot of concern, and now we know for sure his primary concern is milking every dollar he can. We need two doctors to fill out a 2-minute one page form to apply for a guardianship for her. Her primary care physician did it no problem, didn't ask for any $, etc... This guy is insisting he won't do it unless he is paid $300... for a brief one page form. It's pretty disgusting that some people who become doctors are more interested in milking every dollar they can than the well-being of their patients.
There is a lot to unpack in your post. I had to petition a court in Texas for a guardianship and it's not a quick process. I hope that you understand all that is involved in a guardianship. Essentially, you are taking away all rights that loved one has ever had. She will never been able to vote, get married, buy a house, drive a car etc. I totally get that with her having severe dementia she would not be able to do those things anyway. However, doctors take these things very seriously and it takes time to complete such forms. In my opinion, $300 is not an unreasonable expectation. Also, the attorney you are using will have to make sure that your grandmother's best interests are represented. This is done by hiring an attorney ad litem. They want to make sure you are not taking advantage of her condition. When I petitioned the court in Texas all said and done it cost me $10K and that was in 2007.

Their medical degree was an investment. If they are bribing or demanding extra money for doing their job then that illegal. If you have insurance just ask them during an office visit fill out the paperwork but their not doing a good job you can switch.
