Is it negatively affecting her health? People actually eat dirt in other parts of the world. I had a friend who ate clay when she was pregnant.
I have to wonder, as her friend, why you are so determined to "save" her fiance from her rather than help her figure out why she's craving dirt (more than likely her body needs nutrients that she's not getting) and helping her with that.
I'm going to say this - you want to ruin her marriage because there are other routes you can take to deal with her dirt addiction - and you're choosing the one that would hurt her more than help her.
Why don't you deal with your own problems before trying to deal with hers?

I don't think it's a good idea to meddle with someone else's relationship. Frankly, it's not your decision whether or not she tells anyone, continues, or stops. It's her's. You shouldn't go behind her back and talk to her mother either. I believe that you should talk to HER about it. Tell her how you feel, sincerely and warmly, and encourage her to take the correct course of action.

Um, I'm pretty sure that is an actual disorder. Maybe instead of being judgmental encourage her to seek medical help. If she says her fiance knows then he probably does, and either way it is none of your business to interfere in their relationship. And what is the point of calling her mother? Clearly your friend is an adult, though I doubt you are because you sound like a 12 yo, and can take care of herself.
Mind your own business. I pretty darn sure he knows a lot more about her than you do, especially at the point where he's already popped her the question and will to marry her. You need to contact her and talk to her, not anyone else about it. Yes it's weird, but honestly it doesn't sound life threatening so you need to just keep out of it and quit trying to be the savior. Seriously overbearing, hon.
Tell her to go see a nutritionist since she could have an iron deficiency. I know it sounds funny but that's a big symptom of iron deficiency.
I think people probably ate dirt a lot in the past. There's B12 and sh*t in good dirt.
