I would say worse case for North Korea would be a accident with explosion inside North Korea.

India vs Pakistan would be most likely cause for Nuclear weapons used.

You're confused.

The occupying state of Israel isn't a democracy. Also, it has nuclear warheads for it's Samson option - not for deployment against anyone.

The US isn't a proper democracy either - it's a flawed republic. It's nuclear weapons are largely out of date and most of it's weapons systems are Soviet Era. It's more a danger to itself than others.

Russia is largely centred on controlling Russia - it's not interested in expansionism like the West is. Most of it's rockets are unmatched anywhere in the world.

China like Russia isn't expansionist. It doesn't give a shit about what the world wants, only that it gets to make it. It's socially and intellectually what the USA wishes it was.

India is barely a country and you could think of it as a poor mans China. It's sole reason for existence seems to be, compete (and often lose) against Pakistan and Bangladesh and provide a launch ground for British and US interests in the region. It's more a threat to itself than anyone else.

Pakistan is a flawed democracy - not a theocracy. It actually produces a fair chunk of the worlds nuclear warheads for others, it's own stockpile isn't that big. But it doesn't need to be when it's a protected supplier.

North Korea like the occupying state of Israel is not really capable of deployment against anyone. Both share a common goal of annoying others for money, and playing the victim when confronted.

Nuclear war is a distinct impossibility. It's called a "deterrent" as it's too big a weapon to put onto any battlefield and more something to threaten with. Plus with weapons systems now, being mostly electronic, they can be disabled or engaged with before they ever reach their targets.

While Taiwan has never openly stated that they have nuclear weapons they have collaborated on a lot of military projects with Israel (so their working on a nuclear weapon of its own would not be far fetched), specifically long range launchers that would easily have the range to reach 4 mainland china provinces that contain significant manufacturing areas and export hubs and PLA Air-force and Navel bases.

By a large margin, the USA is the country most likely to use nuclear weapons because they are the only country that ever has and the only country that is suggesting that there are occasions they should use them. The second most likely country to use nuclear weapons is Israel. They would use them in a heartbeat except they fear it would cost them support from the USA.

You can survive a nuclear bomb if you are protected from the blast and heat; and, if you can stay protected from the radiation for two weeks.
