Well first of all, this is just a cultural reference to Matrix originally that different groups have adopted separately for different reasons and to varying degrees of "success". I am just going to assume you are talking about the Incel movement here since this could easily refer to half a dozen other topics and groups that has nothing to do with each other since they are all just referencing the same popular movie scene.
Point is that in the movie Matrix the protagonist is presented with two options in the physical from of two pills. One blue pill which if taken would make the protagonist wake up and forget everything about this choice and allow them to return to their normal life. And a red pill which only promises to show the protagonist how deep the rabbit hole goes which is another reference. In the movie the red pill allows the Protagonist to disconnect from a virtual world and wake up in the real world to discover that everything they knew is wrong.
At this point I just want to point out that a lot of people use this blue pill/red pill metaphor because they think its edgy and cool. I personally think they are just childish personally but whatever, moving on.
There will always be an inherent bias when talking about the Red/Blue pill because its built into who is using it. The red pill is framed as the "correct" choice where you exposes reality for what it is where as the blue pill is framed as accepting living inside a lie and derided. This all presupposes that reality is a lie of some kind and also is a very overly-simplified view of things since even in the movie there is a traitor that initially took the red pill but later came to regret it because what they came to learn and experience was not worth it compared to living as normal before.
I guess a better way to view this is not if you accept your current reality or not but rather if you want to accept a conspiracy theory. There are various ways the right wing employs the pill analogy but it generally centers around exposing some kind of false narrative in society. These ranges from how society is secretly favoring women or how 9/11 was an inside job or whatever. Wherever the right wing can identify a difference between perceived reality and their personal beliefs is where they will implement the red/blue pill.
Black pill is a later addition by Incel that goes beyond exposing some kind of conspiracy against men where they are oppressed in society to them accepting that there is nothing they can do to change this hence the "black" in black pill. Its a nihilist belief of sorts.
