I think it depends on how long you have not been to the gym. If it has been awhile then do not over do it. I would do 5 days out of 7, and do at least an hour or two. If it wasn't super long then I would do 5 days still or 6 and possibly and hour more. Seriously, do not over do it or, strain yourself. :)

You go two days on and one day off ratio if ur just starting how long u r there varies depending on ur rest times, the volume of ur workout and how u r feeling there is no set time for being there some people prefer a short workout some prefer a long workout it is all down to personal preference

It really depends on what kind of shape you are in already and your goals. Personally I do 5 days a week for about an hour and a half to two hours.
You don't need the gym. Just do calisthenics and martial arts and you'll be stronger than any bodybuilder.
