Unless you have an allergy to a particular good, it doesn't affect acne; although you may notice an improvement in your skins condition if you exclude less fatty or sugary foods and replace them with more fruits and veggies. The reason being is that high fatty foods and sugary foods can affect blood levels and hormones, although they have to be eaten in excess to really have an effect
Try limiting fried foods and sugary foods(moderation is key) and see if it has an effect
What type of acne do you have? If you have oily skin, that can cause acne irregardless of your diet because it clogs pores and is often ridden with bacteria. So, a combo of benzoyl peroxide + Salicylic Acid can help minimize breakouts, or it should. While cysts can be worsened by oily skin, hormones play a large role with cysts as opposed to bad skin care routine or diet.
No, it shouldn't do, unless you're prone to having a reaction after eating certain foods. And it's been proven that Chocolate does NOT affect your skin...so that's a bonus! Although, some tests were run on a group of Norwegian girls, and supposedly, those who were over weight or had large amounts of stress were prone to worse acne than the others. :)

Eating fast food is not going to give you acne, however the stress and anxiety that may come from it may result in acne. Having naturally oily skin will lead to acne. Sleeping with your face on the pillow will cause pressure acne. Touching your face with dirty hands will cause acne. Acne will cause acne (it spreads). Even soaps that dry your skin excessively will cause acne, because your body overcompensates with oil and that makes you more prone to it.
That's why one of the best treatments happens to be oil-free moisturizers.
Yes, when ever I eat high fat foods I get acne the next day.

It depends on the person, for me if I eat fried foods or foods high in fat/ oil my skin will break out. I usually have pretty clear skin because I drink at least one litre of water a day and I have a low fat diet.
