You can make it look like getting her phone number is not the main reason you want to talk to her. For example, if you approach her and say, "My cousin is running a bake sale this week to help my nephew pay his school fees, if you give me your number, I can update you on the time and place." You could also ask for her number to inform her about a garage sale, or an anime convention, or a movie discount day, or offer to give her a ride to church and ask for her number etc..
If you can't think of a reason to ask for her number, ask for her Facebook or instagram instead. I know guys with very easy to search names, you could create one that anybody can look up like, "I love green rhinos," or "New York biscuit eater..."
Don't overcomplicate it-
Next time you see her, just talk to her like you normally do, smile, and ask her, "Hey, I really like talking to you and would really like to get to know you more. Could I get your number?"
She will either say yes or no, hopefully the former.
Don't overthink it - while chatting with her, invite her to do something with you over the weekend and get her number to stay in contact. If she's interested, she won't think you're a creep and will be happy you asked. If she's not interested, she might think you're a creep, but women can be childish like that.
There's no way to get over the nervousness other than repetition. Keep forcing yourself to talk to not just women, but everybody, and inviting them to hang out/ask for their number.
Asking for a number while a girl is working is tough. You need to find out what she like to do outside of work first (either directly or indirectly) and then try to coordinate a way to run into her outside of work. It's easier to generate social proof from a woman when she sees you outside work.
