My boyfriend said that he thought that he had mono should I be concerned that maybe he’s cheating on me?

He said that he thinks that he has mono “the kissing disease” and I asked him how who else has he been kissing because we didn’t see each other for 3 days and then he said that he doesn’t know and that it’s spread by saliva then he told me not to overthink it

I’m just confused now because why would he say that could it mean that he has been with someone else?

Educate yourself before you come up some crackpot theories. Accusing him of cheating is a big deal... and this your evidence? Your boyfriend 'thinks' he had mono... then you cherry pick one form of transmission and BOOM he's cheating.
You best have REAL good evidence of him cheating if you decide to pull that card. I've dumped a few women on the spot for accusing me of cheating. Many guys would do the same.
