Not gonna lie some guys WILL turn tail and run, but if they do it for that chances are it wouldn't work out anyway. If you tell him you relapsed he will most likely try and get you help, to see a Dr., and finally be able to get back on track without relapsing.
Honestly it will be far worse if you DON'T tell him and he finds out, then he is much more likely to leave, not because you relapsed but because you lied about it and hid it from him.
You need help, he most likely would know that if you told him. Be honest and get the help you need!

I can umderstand you. My weight journey has started when I was 12 and I suffered from anorexia and bulimia for a long time. Then my metabolism got slower and I gained weight. Also, Im suffering from binge eating disorder. I dont know about your parents but mines haven't supported me and they have blamed me and my friends for my problem. I lost all my confidence and gained weight a lot.
