Well she will use a speculum but tell her you are a virgin and she will do all she can to lessen the pain. Not gonna lie I was a virgin my first two times and it does hurt some and is highly uncomfortable, but I like to think better uncomfortable for a minute than having cancer (and yes you can get cancer even if you are a virgin).
She will then take a very thin plastic scraper to just swab the inside of your cervix to detect any cancerous cells, abnormal cells, hpv and such this is what is called a pap smear. After that she will lube up her fingers place them inside and feel your ovaries while pushing on your abdomen from outside as well to see if there are any problems. Some also do a quick anal exam for hemmaroids and whatnot mine does not.
Oh and also before she starts out below she will check random things like a normal check up and also perform a breast exam to check for any growths. Don't worry it's not that bad and after a while you get used to it. Also you may have to pee in a cup mine checks for UTI'S. If you have a male gyno however there will be a nurse in there though even with a female there is one to help hand things and whatnot. Mine is so good I don't even feel anything at this point.
